ANNOUNCEMENT
With great sadness, we announce that Southcoast Allergy, PA will be permanently closing on Feb 28, 2019, due to Dr. Kovacs' failing health.
Our last day to see patients will be Feb 27th.
We appreciate the many years of loyalty and patronage from our patients. For his Allergy/Immunology patients, Dr. Kovacs recommends continuing care with Dr. Michael Neuland or Dr. Carla Hinds. Please stop by the office at your earliest convenience to sign a records release and let us know who to send your records to.

Dear Patient,
We would like to welcome you as a new patient. We feel very fortunate you have chosen our clinic to assist in your medical care. Below you will find information to help prepare for your visit with us.
PLEASE REFRAIN FROM WEARING PERFUME OR COLOGNE TO YOUR VISIT– we have patients as well as several staff members that are chemically sensitive.
If you suspect you have allergies or plan to be skin tested:
In order for us to obtain the most accurate results for allergy skin testing, you will need to stop antihistamines used for allergy treatments 3 days prior to your appointment. Please refer to the list of medications on the "Skin Test Info" page. Patients under 18 years of age must be accompanied by a parent or guardian for the initial appointment. For future appointments, a "Authorization to Treat Minors" form needs to be in place giving us permission to treat without a parent being present (for driving age patients), or designate another party to accompany younger children to our office for treatment.
The initial visit is very thorough and can be quite lengthy. We ask that you plan to be in the office at least 2 hours, so please plan your schedule accordingly. During your visit our physician or physician assistant will review your medical history with you. Depending on your history and symptoms, skin testing may be done. Another procedure called spirometry (a breathing test) may be done if there is any history of chest symptoms. Spirometry helps us to know how your lungs are functioning.
For information regarding payment, refer to our Payment Policy page. For your convenience Visa, Master Card, Discover Card, personal checks and cash are accepted. If you are unable to keep your appointment, please call our office at least 24 hours in advance. There will be a $50.00 no show or same day reschedule fee that will have to be paid before you are seen.
Please complete the following New Patient Forms - 2018 (12 pages) and bring with you to your visit.
Also, a form titled Authorization for Release of Information can be completed and forwarded to your Primary Care Physician or your referring physician prior to your visit. This will allow a copy of your medical records to be released from your physician and forwarded to our office for review.
Our office is located at 4592 E Highway 20, Suite 3 in Niceville (Bluewater Bay). If you have any questions, please feel free to contact our office at 850-279-6520.
Sincerely,
SouthCoast Allergy, P.A.
